🏃🏽‍♂️ Community reintegration & social inclusion

📋 Help applying for the NDIS if ongoing support is needed

📅 Support runs over 3 to 12 months

🌿 Delivered in a non-clinical, culturally safe space

📍For mob 16+, living in West Moreton (Ipswich, Lockyer, Somerset, Scenic Rim)

📄 Referred by a Kambu Health GP, another service—or self-referral welcome!
