Kambu Health Social and Emotional Wellbeing Community Support Program walks alongside our mob as they return to community and regain their strength.
This program includes:
✅ 1on-1 counselling, crisis support, safety planning & care planning
✅ Referrals and coordination with services
✅ Group healing sessions including peer support and carer groups yarning circles
✅ We walk with you over 12 months, starting with intensive support, then ongoing yarns and group sessions
Always delivered in a non-clinical, culturally safe space with strong connections across Kambu Health.
📍 Mob 18+, living in West Moreton (Ipswich, Lockyer, Somerset, Scenic Rim)
Must be referred within three (3) months of discharge from acute mental health care
(Self-referral, Kambu Health GP, West Moreton Hospital and Health Service, or other provider accepted)
